Position Overview'

In this pivotal role, you'll empower individuals grappling with mental illness to thrive within seamlessly integrated environments. Your impactful work involves conducting face-to-face clinical assessments, crucial for admission into transitional adult homes. These assessments, conducted in diverse settings including nursing homes, hospitals but can also occur in the community. Assessments are completed with the use of the state’s electronic system and our own internal system which supports the creation of all determinations and notifications for distribution to the applicable parties, which are pivotal in facilitating comprehensive care for those in need.

This PRN Hybrid role blends remote work from your home office with fieldwork, engaging with patients across your nearby counties. Embracing this hybrid position offers the best of both worlds, allowing flexibility to cover various counties in New York. The role involves 15 - 20 hours per week, including travel to visit patients at their homes or supervised living facilities within New York.

Job Responsibilities

• Demonstrate comprehensive knowledge of psychiatric disorders as outlined in the current Diagnostic and Statistical Manual used in New York.
• Utilize expertise in standards of practice pertaining to determining the Level of Care.
• Apply deep understanding of mental health disorders often necessitating long-term care services and support.
• Exhibit familiarity with public sector services and support systems.
• Proficiently operate Microsoft Excel, Word, and Outlook for effective task management.
• Assess and diagnose psychological and behavioral issues, delivering accurate clinical diagnoses.
• Employ computer equipment and web-based software proficiently for job-related tasks.
• Collaborate with office staff as required to streamline workflows.
• Engage and communicate effectively with healthcare professionals, patients, families, and support networks.
• Exhibit strong communication skills across various mediums—spoken, written, and electronic.
• Demonstrate attention to detail, adept task prioritization, and timely task execution.
• Analyze and evaluate psychological, emotional, behavioral, and physical functioning.
• Develop recommendations for the level of care based on clinical evaluations.

Required Qualifications and Experience

• Minimum of 2 + years’ experience with assessing individuals with mental and psychiatric disorders.
• Must have Unrestricted and Active New York State License as a Registered Nurse.
• Hold a valid and current professional license in good standing in New York State.
• Graduated from an accredited Diploma, Associate degree, or Bachelor’s Degree Program.
• Proficiency in medical record abstracting, understanding medical record organization, medical terminology, and disease processes.
• Possess robust clinical assessment and critical thinking skills.
• Exceptional verbal and written communication skills.
• Ability to collaborate within a team-oriented environment.
• Flexibility and strong organizational abilities are necessary.
• Strong proficiency in Microsoft Office and adept internet/web navigation skills.

Compensation

The compensation for this role is $40.00 to $50.00 per hour.

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level. Thank You.
