Job Description & Requirements Registered Nurse - Case Manager - CM - 
 Unit Information: 
 Number of Beds- 40 
 Number of Staff- 2 RNCM 
 Type of staff: RNCM Day Shift Need 2 RNCM, have 1
 Patient Ratios: RNCM: 1:25, SW 1:40 
 Type of equipment: Computer, HCA supplied cell phone 
 EMR: Meditech 
 List typical procedures performed on unit(s): Discharge Planning Assessments within 1day of admission- face to face and phone if needed. Interqual reviews Frequent liaison contact w/HH, SNF, IRF, LTAC IV infusion, DME referrals and follow up Daily pass down Multidisciplinary Rounds qd 
 Requirements: 
 Must have: RN Case Management experience 
 Familiar w/most of the following MIDAS, Meditech, Care Port Discharge, Outlook, Excel, Interqual, Word 
 Preferred or nice to have: HCA Case Management experience 
 NATE familiarity HCA MDRs 
 Best personality Fit: Flexible, adaptable, quick to learn, team worker, able to ask questions and receive direction 
 Floating Area(s): Occasional float to 4Tele, ICU for assistance 
 Dress Code: Business Casual or scrubs 
 Weekends and on call (if any): Every 3rd weekend.
