The Nurse Practitioner provides ambulatory healthcare services including physical exams, procedures, diagnostics, patient education, and care coordination for employees and their dependents. This PRN role offers flexible scheduling primarily on Fridays and some Saturdays within a collaborative clinical environment. Applicants must hold valid licensure and certifications, have at least three years of clinical experience, and work to support patients' health and wellness.

Premise Health is seeking a PRN Nurse Practitioner or Physician Assistant to join our on-site health center in Groton, CT . In this role, you’ll provide care to client employees and their dependents (ages 2+), helping them get, stay, and be well.

Schedule & Coverage Needs:

  • Fridays: 7:00 AM – 3:00 PM or 8:00 AM – 4:00 PM (weekly or select Fridays, based on your availability)
  • Saturdays: 8:00 AM – 1:00 PM (1–4 per month)
  • Additional: PTO coverage as needed and available

What You’ll Do

  • Conducts physical exams and preventive health measures per guidelines
  • Performs procedures such as injections, suturing, wound care, and infection management
  • Develops patient care plans, records progress, and ensures continuity of care
  • Educates patients on treatment plans, health maintenance, and development
  • Orders and interprets diagnostic tests to assess clinical conditions
  • Collaborates with physicians and healthcare teams for comprehensive care
  • Prescribes or recommends treatments, including medications and therapies
  • Refers patients to specialists or physicians as needed
  • Maintains patient health records ensure accurate and up-to-date documentation and records
  • Educates and coaches nursing staff on best practices
  • Performs other duties as assigned

What You’ll Bring

  • Maintains state RN licenses/certificates to practice
  • Maintains Nurse Practitioner licenses/certificates to practice
  • Maintains State Prescriptive Authority (if applicable)
  • Federal DEA license (matching each state of practice as applicable)
  • State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
  • Current national certification (ANCC or AANP required)
  • Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
  • Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
  • At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting

Premise provides its reasonable and genuinely expected range of compensation for this job of $78-$80 per hour. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
